Hina:

Aap na apna symptoms nahin bataye.

Anyway, progyluton aik hormonal pill hai jo irregular menses mein prescribe kee jaati hai, aur iska illawa bhee kae different cheezon kay liya prescribe kee jaati hai.

Agar aap apna symptoms aur lab results mention kar dain to aap ko zyada behtar bata sakata hoon.

Re

AOA sir...how r u??

last time i wrote abt my hsg...well my doctor said it was perfect flow in tubes.. after that she prescribed clomid for me...50 mg for 1st month,,100mg for 2nd,, 150mg for 3rd.. to be taken on CD 2-6... and she also gave me norethisterone accetate (primoult) to be taken on CD 16-25, also for 3 months.. she said to come after 3 months (if i dont get preg by clomid then).
so i took 50 mg clomid in june, clearly it didn't work and got my period on time.
now taking 100 mg.. im starting to use opk's this month so as to know whether im ovulating or not..
if this month fails then maybe i'll go for follicular tracking or day 21 progesterone test..
meanwhile trying to loose weight n also trying to maintain a positive mental attitude...

the thing i wanted to ask is that my friend (also having pcos)... was trying 4 baby for more than 2 yrs...her doc just gave her metformin, which she took on and off (not regularly)... after some time her doc gave her clomid 200mg for a month,,she took it and she ovulated really late that cycle (CD 26) and got pregnant that cycle... can i take clomid for more than 3 months and higher dose?? i heard it causes cysts....

just wondering whether u approve of my coarse of treatment...
thanking you
:)

Buttercup:

Walaikum Salam,

I am fine, thanks.

How about you?

The one major concern that I have about your treatment regimen is that Primolut should NOT be given in suspected early pregnancy. It is actually contraindicated in pregnancy, and if you look at the medication leaflet, it should clearly say so. Therefore, prescribing primolut on CD16-25, right after you have been given clomid to ovulate and conceive, is baffling. Besides, clomid itself helps not only with ovulation, but also with producing the appropriate hormonal changes to support early pregnancy. Basically, it stimulates the ovary to produce increasing amounts of progesterone after ovulation, which then helps with the successfull implantation of pregnancy. I know, previously your progesterone levels were low on day 21.

The second concern that I have is that clomid use is associated with approximately a 10% chance of cyst development or ovarian enlargement, and requires monthly exams before the next cycle is prescribed. Unfortunately, not all doctors follow this . It seems like you are not getting that either.

I am glad that you did decide to start using a OPK, it would be very helpful to you, and would also help you in timing the intercourse. As I have always mentioned, it is of prime importance for you to know whether you have been ovulating or not, and OPK is a good, cheaper and more convenient alternative to follicular tracking, though certainly not as accurate, to find out whether you are ovulating.

You are only on your second cycle of clomid, so the good news is that time is on your side. Please know that about 75 % of women who will get pregnant on clomid, will do so within their first 3-4 cycles, after that success rate diminishes, but there are other options as well.

I know it is tempting to use a higher dose of clomid, particularly when your friend has just conceived on a higher dose, but please know that if you are ovulating on a smaller dose ( but not getting pregnant for one reason or another), increasing the dose would not help much. Therefore if the OPK predicts that you will ovulate, than going up much higher on the dose might not be very beneficial, and there are more side effects with the higher dose.

Some doctors use a combination regimen of clomid and HCG injections with good success rates as well. But that certainly is something that is upto your doctor to decide. And your doctor may also want to consider adding metformin to your regimen.

And yes, you can use clomid for upto 6 months, even though as I said above, the likelihood of you getting pregnant is greatest during your first 3-4 ovulatory cycles on clomid.

Dear sister, I know it could be very frustrating being in the situation that you are in, but please know that infertility treatment is sometimes time consuming, and can even take upto a year or more. So it is imperative that you keep a positive mental attitude, which you already are, and always think positively. And certainly continuing to exercise and loosing weight is going to further increase your chances. By the way, how much weight have you lost so far?

I certainly wish you the very best.

RE..Psychdoc

AoA... im also well..

i know that primoult is not gud in early regnancy (read in the leaflet).. but im really confused that why my doc gave me, knowing that im TTc.. its maybe bcoz of my irregular cycle.. i think my doc would agree if i say that i dont want to take it,,after all that is wat mostly drs do to see wat the patient wants.

on the other note, can i skip primoult this cycle?? and if periods delay again,,then take it for 5 days to induce period (ofcourse after testing for pregnancy). im really confused wat should i do now as i have already done 2 cycles of primoult.

yes doc,it certainly is very frustrating...not to mention the society pressure and disapproval,, still trying to be positive.. i have lost 3 kg in a month so far, will do more effort....and after clomid, i will b doing injectibles, and hsg trigger shot..and then iui's...thats wat my doc said..

thanks a lot for yr guidance and patiently explaining everything..

Buttercup:

Walaikum Salam.

And you are always welcome.

Quiet often, primolut is prescribed to those women who do not have regular menses, and primolut is prescribed to them to start a period, so that clomid could then be prescribed either CD 3-7 or CD 5-9. Are you sure this is not what your doctor meant? You may want to double check with your doctor again and ask why the primolut is prescribed at CD 16-25, particularly when you are trying to conceive. Primolut is also prescribed from CD16-25 in a variety of gynecological conditions, like dysfunctional uterine bleeding, in order to regularize the periods. I would highly recommend you to check with your doctor regarding why the primolut was prescribed in your particular case.

Good job with the weight loss.

Once again, you are very welcome, and best of luck.

RE..Psychdoc

AOA... how r u?? and ramadan mubarak to u..

i have been trying to get an appointment wid my doc after u suggested to check about primoult..ut unfortunately shes out of country for a week and the sooner i'll get an appointment wid her is 2-3 weeks later...

so i asked her nurse instead...obviously she couldn't do anything abt it except saying that primoult is not a contraceptive..u can use it, it will regularize yr periods.. but for me..pregnancy is much more important than irregular periods...

i started doing opk this month from cd 10(as instructed),,and i still have not got a positive yet till cd 16...but hopefully i will InshaALLAH, as the line is darkening v slowly... now i have decided to skip primoult this month and wait to see if my body ovulates or not... is it possible not to ovulate while taking clomid (100 mg this month)..i have read that pcos can delay ovulation even with clomid...so i was just wondering that....

is it possible doc,,that im ovulating late..and by the time my body ovulates,,i have already completed my primoult coarse of 10 days, considering that important days are up... and in this confusion i get a period instead ??bcoz primoult is a harmone,,surely it doesnt wait for the egg to come out to cause a period??
thanks...
AH

Buttercup:

Walaikum Salam.

I am fine, thanks, how about you. And ramzan mubarak to you as well.

Sorry I had to change the category of your question, because the topics in the married women problems can't be opened for some reason, but this category seems to be working fine.

I will have to agree with you that you did the right thing in choosing not to take primolut for this cycle. And yes, it is possible that ovulation may not happen with the 100 mg of clomid, though chances are it will, inshallah. And yes, ovulation can be delayed in PCOS as you said.

One important thing fro you to consider here is that a woman is most fertile from around 4-5 days BEFORE ovulation, to about 24 hours after ovulation. Therefore do not wait for intercourse until you ovulate, but start alternate days intercourse with your hubby well before the expected ovulation. This would give you the best chance for getting pregnant.

I am not entirely sure what you meant to say in your last paragraph, can you clarify?

All the best.

RE

WAS.. im ok thanks..

well,,i just got yr reply now..and thanks for that! its a huge relief to know that i did right in skipping primoult this month... as for me,,its cd 25 and i think im unsure of whether i ovulated or not...trying to get to know opks for now...and yes we are having intercourse on alternative days anyways...

i wanted to ask previously that,,does primoult induces period without ovulation? and it is possible to ovulate white taking primoult in cd 16-25??

thanks a lot for ur help... thanks for being our virtual doctor :)

Buttercup:

AOA. Sorry for the late reply, and thank you for your kind words. And as always, you are very welcome.

Well, I certainly hope that by this time you would have ovulated.

Yes, indeed , it is very possible that you could have ovulated between CD 16-25. But please bear in mind that primolut primes the endometrium for menstruation, ie it brings about the necessary changes in the lining of uterus, making it possible for menstruation to occur within 2-3 days after discontinuation of primolut.

Yes, you did absolutely the right thing in not taking primolut after your clomid cycle this month.

I certanly wish you all the best and hope that your long wait would be over soon, inshallah.

RE..Psychdoc

AOA doc.. how r u??

long time since last reply.. so i took 3 rounds of clomid, no pregnancy so far..
from last 2 cycles,,i have been using opks to detect my ovulation, and it happened on cd 26-27.. so it means clomid make me ovulate..
now my doc told me to come after 3 cycles,,then we will do injectibles.. and those cycles will be monitored..
so my concern is that,,shouldn't i do just one more cycle of clomid (200 mg this time) without my doc's advice?? afterall,,clomid can be used till 6 cycles, right?

apart from that,,my mother in law took me to hamdard hakeem,, n he gave me herbal medicines for pregnancy..he explained me well that these r for ovulation..so i had to take them for 3 months (mother in law was with me).. they have hamdard laboratories medicine n i have heared good things abt them... i asked a lot of ppl,,n mostly say that u coulod try,,they have no side effects...
so im really confused now :$
since now,i have been put on medicines,, i feel that i should try some more medicine b4 moving on to injectibles...because i dont think doc will turn back to medicine if that also failed..
in all my desperation...i did istikhara..and in my dream last nite (day 2) i saw myself holding a packet of glucophage (which i already am taking)...lets see if i see anything tonight as weelll...

so i need yr advice that what should i do... 4th clomid cycle (200 mg)... or hakeem medicine for 3 months...or move on to higher treatments like injectibles,,iui's...etc etc

JAZAKALLAH for reading it all... i hope to get a good advice from u

Buttercup:

Walaikum Salam,

I am doing very well, alhamdolillah. Thank you! How about yourself?

Yaa I know it's been a while since I heard from you, and let me tell you that it is great news that you are indeed ovulating. So basically, it means that you are one step closer. Again, that is very good news indeed, mashallah!!! And I am also glad that your doctor has decided to start you on glucophage(metformin).

OK, so let's discuss your questions and concerns. But before I do that, I would encourage you to read my July 14th answer one more time, as it may clarify a few things for you.

Anyway, regarding the hakeemi medications, I must admit that I do not know a lot about them , except that as compared to Western medications that are developed after extensive amounts of research has been conducted before they are even allowed to hit the shelves, herbal /hakeem meds do not even come close to the same quality control methods that Western medications are subjected to, and the statement that hakeem meds are free from side effects is certainly not a statement that is based upon reality. The difference is that with Western meds, we know and clearly predict what their side effects are, but with herbal meds that is not the case, because there is just not enough research involved in their production, and there is no or very little ongoing monitoring and scrutiny involved either.

While I certainly do not wish to come across as someone who is insensitive and disrespectful towards the traditional/Eastern methods of treatment, but I also feel that it is my obligation to tell you what I feel to be the truth and what is best for you. So if you ask for my opinion whether you should take these herbal meds, my answer is NO. But that's just my opinion.

Regarding whether you should go for one more cycle of clomid at a higher dose or go for injectibles, I guess there is no right or wrong answer here. Either approach could be tried, but if your doctor wants you to be on injectibles, I think you should follow your doctors advise.

It is a very good thing that you did istikhara, as certainly this is what our religion recommends us to do whenever we are faced with a dilemma. I am sure that you would get a clear direction as to which way you need to go, and may Allah make that decision the best decision for you, Ameen.

My dear sister, I know that it is confusing, baffling and frustrating for you, and I just hope that I did not increase the level of your confusion by anything that I said. Instead, I hope that you would be able to make a more informed decision regarding what you need to do. As far as I am concerned, I am 100% in favor of clomid and glucophage, either with or without the injectibles.

As always, my prayers and best wishes for you.

Please take very good care of yourself.

CheersJ
