Tests Done For Fertility assessment: Some basic tests I will explain a bit or more… Semen analysis(semen D/R) of male is done first when couple has difficulty in conceiving….. method: it is asked that male refrain from sex/masturbation for 3days and then submit semen sample in lab by mastrbation and sample preferably taken in lab .. if at home their container is taken and reach lab within 20min.... mostly if abnormality is found... 3samples 1 week apart are to be taken... WHO criteria of Normal semen …(for fertility) Some basic points… Colour : grey or white(should not be pink or red) Volume : 2-5ml Sperm count : 20-40million/ml(minimum 20million/ml) Alive sperm : atleast 75percent Motile active : atleast 25persent Motile sluggish : atleast 50percent Normal shape and form : atleast 30percent PH : 7.2-7.8 Pus cell :2-3/Hpf if present considered just normal if Ph is okey…otherwise not No antibodies be there… Female basic tests: Hormones level…(in blood) Level of FSH and LH be checked on 2nd day of menses bleed Level of estradiol and testosterone be checked on 2nd day of menses bleed Ratio between LH/FSH is taken on 2nd day of menses Progesterone level be done on 21st day of menses cycle LH may again be counted on 10th-12th day of menses cycle to know if it increases or not… In urine LH level is done at 12th day of menses to know it increases or not… Ultrasound: On 10th day of menses cycle… Ovaries size , shape, volume is seen…ovaries volume must be below 10ml and no cyst be there and increased follicle of 12-18mm be there and one follicle be so big upto 10-24mm be there normally Uterus is seen …any defect in position or shape or if any mass inside uterus that prevent pregnancy… Better is the ultrasound done through vagina(TVS….transvaginal sonography)…..…. A bit costly…. Some other test: even secretions from mouth of uterus are taken and mixed up with sperms and seen under microscope to know that if there is problem in their mixing up naturally… Tubes test:to know tubes are open and can egg pass from them or not.. Hysterosalpingography: In this test, doctors use x-rays to check for physical problems of the uterus and fallopian tubes. They start by injecting a special dye through the vagina into the uterus. This dye shows up on the x-ray. This allows the doctor to see if the dye moves normally through the uterus into the fallopian tubes Laproscopy: a camera is entered into abdomen and ovaries , uterus and tubes are seen and area around them is seen to know if any abnormality in surrounding area….. Treatment of infertility: Definitely the underlying cause is to be treated.. In men: Doctors often treat infertility in men in the following ways: Sexual problems: If the man is impotent or has problems with premature ejaculation, doctors can help him address these issues. Behavioral therapy and/or medicines can be used in these cases. Too few sperm: If the man produces too few sperm, sometimes surgery can correct this problem. In other cases, doctors can surgically remove sperm from the male reproductive tract. Antibiotics can also be used to clear up infections affecting sperm count. In Females: Medicines: Medicines are given mostly 1)mostly in hormonal problems leading to disturbed menses 2) ovarian cyst …. Or polycystic ovaries(ovary with multiple cyst) 3) infections inside pelvis/abdomen 4)ovulation problem…. One thing is to be remembered these medicines are to be taken for months and regular follow up by the doctor and regular ultrasound is necessary and never loose hope…. Surgery: 1) when tubes are blocked 2) any mass or tumor in uterus or ovaries that donot get treated by medicines.. 3) endometriosis: when uterus lining is also present outside uterus 4) ovary cyst that not get treated by medicines.. Arificial(assisted ) reproductive techniques: These are the methods by which couples are helped out to conceive when they fail to conceive naturally….. IUI(intra uterine insemination) semen is first concentrated and injected high in uterus …. Mostly for couples with low sperm count or motility but female partner is healthy.. IVF(in vitro fertilization) or test tube babies… when problem is with male or female or both…. Mixing of good quality sperm and egg is done outside body and then placed in mother uterus..
