IUI andIVF

  Infertlity treatment:assisted reproductive techniques(masnoi tareqeey se couple ko pregnancy karwana)

 IN UTERO INSEMINATION(IUI) OR INTRA UTERINE INSEMINATION  (male partner ke semen ko uterus yani bachey dani ke ander dalna masnoi (artificial) tareeqey se  bagair intercourse(natural sex) key.)

Ye method un couples ke leye ha jin mai wife mai koi masla nai hota pregnancy karne ke leye magar husband mai problem hota he. Wo problem kuch bhi ho sakta he jaise

1)      male impotent ho yani sex ke salahiyat na rakhta ho

2)      uska semen bohat kam ho

3)      uske sperm kam hon  number mai ya kharab hon ya sperm ke harkat kum ho

4)      uske semen mai koi aisa chemical ya toxin ya infection ho jo sperm ko kharab karta ho ya aurat ke uterus mai ja kar pregnancy na honey dey.

Iss method ke leye bohat zarori ha k aurat ke tubes (uterus aur ovary ke beech mai nail jo inn ko aapis mai milati ha wo khuli hon yani thek hon) patent hon/open hon.

Female ka monthly cycle periods ka thek ho aur wo aik egg atleast 1month mai apni aande dani(ovary) se produce karti ho . ye check karne ke leye  ultrasound keya jata he aur hormone ke level daikey jate han.

Procedure:

1)      husband ka semen leya jata he . phley usko btaya jata he key 3days tak sex nai karna phir masturbation ke zareye uska semen leya jata he

2)      semen ko prepare(tayar) karte han laboratory mai yani usko concentrate(thick) kya jata he by swim up technique aur cool ya freeze kya jata he.

3)      Iss tarah sirf good sperms reh jate han semen mai aur wo chemicals jo semen ke ander hon aur sperm ko nuksan daiti han unke tadad khatam ho jati ha.

Ab wife ka  monthly cycle closely observe kya jata he by ultrasound and hormones level

1)      jab wife ke ovary ke ander eggs size mai increase kartey han tu unko ultrasound ke madad se observe kya jata he.

2)      jab egg release hony ka time aney lagta he tu husband ka phley se leya huwa aur tayar kya huwa semen aura ke uterus ke ander needle ke madad se dala jata he. Vaginal opening se  syringe under daal kar semen uterus mai dalte han.

          3)Iss tarah good quality sperms jate han uterus mai aur jab egg release ho kar uterus mai ata he tu fertilization(baby formation) uterus ke ander ho jati ha.

Advantage

This is a simple technique and less costly.

Disvantage

Failure rate is high.

IVF    in vitro fertilization (test tube babies)

Iss method mai eggs aur sperms ko female ke body ke bahir mix kr k fertilization kei jati ha aur phir eggs ko uterus ke andar rakha jata he.

For infertile females this method is mostly used yani wo females

1)      jin ka monthly cycle normal nai ha

2)      unkey eggs produce nai hote monthly

3)      jin females ke tubes(uterus ke nail) band han

4)      ya wo females jin ko aisi infection ha k wo uterus ke ander fertilization nai honey daita.

5)      HUSBAND may or may not be infertile. Agar husband bhi infertile ho tub hi ye method use kar sakte han.

Procedure

1)      husband semen is collected and made good by the technique same as IUI

2)      females ke egg khud drugs aur hormones day kar release karwaye jate han ovary se

3)      female ko menses cycle ke 2^nd day par drugs dei jati han aur atleast 10 day tak drugs use karma hoti han. Ye cycle of drugs may be repeated for 2-3 cycle of menses if required.

4)      Phir ultrasound ke madad se ovaries ko monitor kya jata he jab eggs size mai barey ho jate han tu vaginal opening mai syringe dal kar 3-4 eggs ko baher nikala jata he.

5)      Inn eggs ko husband semen ke sath body temp ke mutabik aik glass plate mai for 2-3 days rakha jata he yahan tak ke ye mix ho jayen(incubated at 37 degree celcius for fertilization) aur inka size mazeed increase ho(6-8 cells size)

6)      Then these eggs (fertilized) atleast 3 of them are put into female uterus to develop.

Advantage

   v.high success rate

   3 or more eggs are placed inside uterus atleast 1 of them do survive

   females with no eggs production can take donated eggs

disadvantage

 costly

more than 1 baby like twins and triplet may for

ovaries of female r too much affected by drugs.

IUI    vs     IVF

1) female has to be fertile and completely normal in body functions (producing eggs and tubes open) related to reproduction for IUI male being infertile.     In IVF however female may not be fertile or her eggs can be produced by drugs  and even her tubes can be blocked , male partner may or may not be infertile.

2)in IUI fertilization(baby forming) occur inside uterus like naturally.  In IVF fertilization  is carried outside body and fertilized egg are kept out for sometime till they get big

3)in IUI single pregnancy occurs most of the time   .  in IVF more chances of twins and triplets.

4)in IUI only semen of husband taken and processed and put into uterus .  in IVF however the female eggs are also to be taken out of body and semen also taken and processed

5)natural ovulation(egg release) is used in IUI. In IVF egg production is introduced by drugs or hormones.

6) ICSI is step ahead of IVF same as IVF bt sperm are injected into egg by artificial means. In IVF both are kept to mix naturally. ICSI is more effective.

IUI and IVF both are allowed in islam IF husband semen is used none other than that.IVF is much costly and time consuming than IUI bt more successful.

mera sawal yeh hai keh kia masturbation ke ilawah aur bhi koi tareeqa hai semen lene ka, especially impotent person ka semen kaise lei.n ge?

impotency and artificial conception

Couples with male partner impotent that are helped by assisted reproduction techniques are of following types.

1)male partner unable to maintain or achieve erection for such time that is required  to discharge into vagina or ejaculate inside vagina. Yani erection bhi ha aur ejaculation bhi but not inside vagina but outside due to early discharge( yani husband ka penis inti kam dair ke leye hard hota hekey wo vagina ke baher he discharge ho jata he) this state is also impotency with respect to sexual relation……… in this case we can take semen by masturbation

2) penis gets hard or erect but no ejaculation occurs…this is another type of impotency ….. this is basically retrograde ejaculation matlab body ke ander he discharge ho jana aur opposite discharge matlab baher ke bajaye ander.in this  a surgical procedure is done that is transurethral cytoscopy(penis ke hole se aik tube like instrument se ander jaya jata he ) kunke in these cases wo tubes blocked hoti han jo sperm ko baher lai kar ayen at time of sexual excitation balkey sperm ander opposite direction mai ja kar urinary bladder mai discharge hotey han and urine ke time par pass hotey han. So iss instrument se ander ja kar wo blocked tubes kholi jati han n then semen comes out.

3) in cases other than above two like inability to erect even then first TESTICULAR BIOPSY is done. Yani testes ka tissue lai ker microscope ke under daikha jata he lab mai ke sperm bun bhi rahe han ya nahi. If these are formed then jab ye sperm mix ho jate han fluid from prostate(male gland inside body that produce fluid for nutrition of sperm) ke sath tu body ke ander aik jaga par ye semen rehta ha/store hota he for 1-2 day that place is called epididmydis uss jaga se semen can be collected by surgery.

Mostly cases of impotency that come for artificial technique are type1 I mentioned n then type 2 its rare for type3 cases to be in a marriage relation unless some trauma in accident occurred and male previously married and potent is now impotent and want children.
