what i know is in CRF one Develops uremic pericarditis and cardiac failure so gallop rythm appears and we need to treat the underlying cardiac failure

i had my Long case of CRF and after usual questions and uremic enceph and jumpy leg syndrome all i told was asked the above question that i did reply as i told But seemed like wasnt enough.Will you please Tell me mechanism because i am really worried and also is this a question for a final yr exam?and treatment Of Gallop rythm in a patient with CRF (i answrd trtmnt of cardiac failure and CRF)

Dear Waterr,

Come on, that was so simple to answer. But first what I would like to tell you is that Unfortunately in Pakistan Professors can ask any question. Being a final year student this is not possible to give answers of all their questions. Also they don't expect all answers from the candidate. Passing and Failing depends upon the basic questions and answers. The difficult questions if answered correctly would definitely increase the marks. In simple words, the above question would be or not would be a question for Final Year Exam. Well, nothing to worry about since it was a difficult question at final year exam therefore; perhaps they were expecting the answer of it from you since your viva was going on very well, And perhaps they wanted to give you distinction. I am sure however; like a brilliant student you would be able to get good marks.

Answer Of The Quarry:

In CRF a patient may get Anemia due to the lack of Erythropoitin production since it is produced in CRF. Therefore; The anemia developed in CRF is called as Anemia of Chronic Disease. Once a patient develops Anemia in CRF he would enter in hyper kinetic state. If you remember, S4 sound is produced in hyper kinetic state. When the S4 sound is produced then it may make summation with S1 sound and heard like a Gallop Rhythm.

Thanks a lot that u cleared my confusion , it was on my mind since last 10days and was thinking that i shud or shudnt ask u

if i tell u honestly my whole obs viva was on anemia(easiet ever viva) cuz to the Obs proffessor i lukd anemic like anything and after that easiet viva he asked me what u think u ave major risk (major organs) esp that occur as pathological event in anemia and physiological event in preg n i replied xactly the same Hyperdynamic state-- high CO failure

but may b thats in CRF my patient was about to go in enceph n my whole concentration was there and his legs which were restless and on top of it Davidson mentions Uremic pericarditis as cause of Cardiac failure in CRF and never mentnz anemia as cause of it(just mentn anemia in CRF) AND i Cudnt place 2 plus 2 on my own :(

i am sort of contended cuz i ave studied medi the most cuz i like it n want to carry on with it in future Inshallah...
