Sciatica: What Are The Treatment Options?

Dear Doctors.
I am Afzal From Hydrabad. I am Married .
I have a problem  i am feeling pain in my hips only a side , thigs and muscles andpain run to down to the left leg . Pain is only Left Leg . I can't move easily from one side to other during sleeping. I can't walk propelry I can't pick weight.  
I am feeling this pain from 8 month first little bit but now So hard. I think and
also Doctor and Hakeem Says this is Sciatica(Arqun Nisa). Doctor says Sciatica 
pain be in several cases Some nerve is Push in my body. I saw Arthow Doctors. 
Right now A neuro Doctor . He recommed me Mri of Lower back.
This is my Mri Report.

"MRI of the Lumbosacral Spine"
"Scanning Protocol"
Multiplanar images were obtanined according to deparmental protocol.
Findings:
Loss of normal lumber lordosis most likely due to muscle spasm.
Disc dessication with slightly reduced disc height noted at L4/L5.
Diffuse disc herniation with central extrusion and mild downward migration noted at L4/L5.
There is associated mild facet joint arthropathy. There are causing compressionover thecal sac and narrowing of both neural foramina more marked on left as compared to right.
No evidence of spinal canal stenosis is seen.
The caudae equina and filum terminale are normal.
The paravertebral soft tissues are normal.
The vertebral bodies show no abnormal signal to indicate bone marrow replacement.
Conclusion:

Pain in Hips and Run down to the Leg

i think exercise is best for you and your doctor will advise you some exercises othervise surgury is best way if your case is very serious or you are faceing severe sciatica pain. Fallow your doctor.

Sciatica:What Are The Treatment Options?

Physiotherapy is one of options for you, Opt for it regularly and if you go to a good physiotherapist he will guide you about excercises that you can do at home

OZONE therapy (medical ozone) is also an option but you need to find where its done its been practiced in private set up and has shown good results uptil now.These are radiation of oxygen mixed with ozone bt DONOT harm like other radiations

surgery , Only a neurosurgeon can tel whether u may go on only with conservative therapy or opt for surgery and one thing to be known is the ultimate cure(of sciatica) is operation to fix vertebrae with screws BUT most People Go on with Conservative (physiotherapy) life long and be fit and fine

u must do some precautions :

never lift heavy things

body posture .. Never ever lift things at right angle from floor , first sit and then lift..

not to jump out of bed in morning or after lying down, ratheq sit and align urself then slowly stand out of it

there are many other precautions that a physiotherapist may guide you.

My doctor says you should be Operate. Your nerve is pushing by disc.

Afzalela , honestly no ône on net can say u go on or donot go with surgery however i can give u a true advice

1) take another good neurosurgeon Opinion , I dnt Knw Abt Hyderabad but in Lahore there are Top most Neurosurgeons (nt allowd on forum to tell hospt or doc names) but its not hard to find Good doctors, Must take a second Opinion

2) though any surgery On Spinal cord is a major surgery But Most Neurosurgeons do Consider Sciatic Nerve Surgery an Easy One Compared to what they are dealing with usually

for disc slip patient like you its impt to weigh the chance of total prolapse(leading to paralysis) to the risk in operation

most people are much relieved after surgery however many do carry on with conservative treatment with rest of life
