Names Of Medical Investigations To Pin Point Male Infertility

meri wife last 16 Months say conceive nhi kar saky... Main nay wife ka 3 doctors say checkup karwaya hai... Sab Doctors Ultar Sound Karwaya... Reports theek hain.. apny bhi 4 dafa test(Semin Analysis) karwa chuka hoo woo bhi theek hain...

Mere Question Ya hai kay Male kay aur keya keya Medical Test Ho Sakty Hain(Semin Analysis Kay Alawa) jin say Infertility kay reson ko pin point keya ja saky... please agar detail (Name of the test and description) main information ho to batain...

My wife is not able to conceive since last 16 months. Three doctors have checked my wife and done ultrasounds but there had no abnormality. Similarly I got check up four times and semen analysis has also been done that also turned out normal.

My question is that what are the further tests for male beside semen analysis to see infertility is there or not. Tell me in detail please.

Well dear following things should be considered while any couple is willing to have a new born.

  • According to a medical point of view if there has been no good news with in 3 years of marriage then the investigations for in-ability to have a child should be started. Before that a couple can have the investigations but according to medical point of view this is not recommended.
  • Secondly it may happen that the husband and wife are absolutely normal then what is the hurdle in preventing the pregnancy? Are they practicing the correct technique during intercourse? Are they aware of it?
  • There are a lot of conditions that may mimic infertility either from husband’s or wife’s side. Before thinking about these problems it is medically advised to meet above mentioned two criteria first
  • Now since 3 years has not been elapsed till so far therefore don’t worry about fertility and now its time to check whether or not you and your wife have been adopting correct positions and techniques. For your and other people’s information I am writing down few important points in this regard:
  • There are only six days per monthly cycle during which a woman can conceive
  • How to calculate the days of fertility: The eggs are released from ovary of the female alternatively from each month. For those who have regular 28 days cycle of menstruation, calculate day 14 from the first day of last menstrual period. This is the time of ovulation. Now since the day 14 to the next 6 days is the fertile time and intercourse during this period can give fruitful results other wise intercourse whole of the month will not result in pregnancy. Those women who have prolong menstrual cycle but its regular then subtract 18 from the total length of cycle and the number obtained is the day of ovulation. For example a woman has 35 days regular cycle then subtracting 35-18 = 17. Therefore day 17 after 1st day of last menstrual cycle is the day of ovulation and sex from this day till further six days would be fruitful. Other signs of ovulation are increase in body temperature and change in the consistency of discharge or mucus from vagina
  • Make sure of putting pillow beneath the wife’s buttocks before proceeding so that the semen must enter deep inside the vagina.
  • Using a lubricant is advised during sex but remember most of the lubricants are spermicidal hence they make semen useless. Better to use egg white or Vaseline as lubricant.
  • After ejaculation don’t let wife to stand up and advise her not to get up till at least 20  minutes other wise the semen could come out.
  • Both partners should eat a well-balanced diet, including foods high in anti-oxidants (e.g. fruits and vegetables) and vitamins, and consider eating less seafood, as it is associated with increasing blood levels of mercury, which has been linked to fertility problems
  • Stop smoking if doing. Tobacco affects cervical mucous in ways that may obstruct fertilization. Smoking also reduces sperm count and sperm health.
  •  Sleep better: Sleep deprivation results in lower levels of the hormone leptin, without which ovulation may be affected
  • Quit Caffine if using (or at least cut back). A recent study has shown that consuming more than 200 mg of caffeine (approximately two cups of coffee, but it can come from other sources like tea and soda as well) on a daily basis during the first trimester of pregnancy may increase the likelihood of miscarriage. Since you don't know instantly when you're pregnant, you may want to consider quitting caffeine or reducing your intake to, say, a single cup of coffee per day.

Sir Thank you very much for a detailed reply... i totally agree with you in every factor except one...

Sir according to my information...that if a couple is not able to conceive for 12 months then that couple is considered infertile...

Reproductive endocrinologists, the doctors specializing in infertility, consider a couple to be infertile if:

  • the couple has not conceived after 12 months of contraceptive-free intercourse if the female is under the age of 34
  • the couple has not conceived after 6 months of contraceptive-free intercourse if the female is over the age of 35 (declining egg quality of females over the age of 35 account for the age-based discrepancy as when to seek medical intervention)
  • the female is incapable of carrying a pregnancy to term.

Sir rest we follow each and every step that you have mentioned... from very first day...
I do not smoke and also have not taken a single cup of coffee after merrage and also stopped eating chocolate after merrage because some told me that these are not good for fertility...

we even use Ovulation test strips to pin point fertile days... but now we are using basal body temperature charting technique as strips are a bit expensive...

Sir it is requested that please give us information about the investigation both male and female... as 3 years seems too long... i hope you understand

Well it is right that the couple should be concerned after one year if they don't conceive however up till three years if they are not conceiving without protected sex then they will not be called infertile. We have a lot of examples in our society where childbirth has happened after many years. After all being a Muslim we should also pray to God in this regard as well. You have asked different tests for male infertility. If your semen analysis is normal then you are 100% alright. No further investigation is required. However if you want to have further satisfaction then following things are required.

1. Complete physical, medical and surgical examination by a doctor in person in order to find out any because chronic disease, pelvic injury, childhood illness, abdominal or reproductive organ surgery, recreational drug use, and medications can affect fertility.

2. Physical examination may detect testicular irregularities (e.g., vericocele, absence of vas deferens, tumor), evidence of hormonal disorders (e.g., underdeveloped reproductive organs, enlarged breast tissue), or evidence of testosterone deficiency.

3. Sperm's ability to swim through cervical mucus and bind to and penetrate an egg. The post-coital Sims-Huhmer, or sperm-mucus interaction test, examines whether the sperm are able to swim through the female reproductive tract. This ability is referred to as forward progression. In the middle of the menstrual cycle, the cervical mucus becomes watery. Intercourse is recommended during this time, followed, the next day, with an inspection of the mucus to determine if

  • enough semen was delivered to the cervix;
  • Sperm are healthy and do not show large numbers of clumped, motionless, or dead cells; and
  • Sperm are swimming energetically through the cervical mucus.

4. The sperm penetration assay (SPA), or sperm-oocyte interaction test, examines the ability of sperm to penetrate the egg by combining it with a hamster egg. The immunobead test looks at semen for the presence of antibodies that damage sperm.

5. Post-ejaculation urine analysis may identify diseases that affect fertility, such as kidney disease, diabetes, and repeated urinary tract infection (UTI).Blood tests identify disorders that impair testosterone and sperm production.

In case of female infertility the following things should be considered and investigated.

 

  • Is the patient ovulating?
  • Are the conditions for implantation adequate?
  • Is the morphology of the uterus and the tubes normal?

For Ovulation following things are observed:

  • Basal body temperature. It usually raises at the time of ovulation. If body temperature raises at the time of expected ovulation then it means the woman is ovulating
  • Cervical mucus: At the time of ovulation the cervical mucus changes in consistency and that also gives evidence for it.
  • Transvaginal sonography (ovarian follicles): This is an ultrasound to see directly in ovary for ovulating follicle presence or absence.
  • Pituitary and ovarian hormones assays: Hormones responsible for ovulation are assessed
  • Laparoscopy and direct observation of the ovaries: Via camera ovaries are visualized

Clinical evidence of readiness for uterine implantation

It means that the uterus is now ready for getting the implantation of the product of sperm and ovum that is zygote which is formed by fusion of sperm cell and ovum.

Basal body temperature: Again here the basal body temperature is considered since it raises as well during preparation of uterus to receive the zygote for implantation.

Transvaginal sonography: Again it is done to see the thickness of the endometrium.

Plasma progesterone assay: Progesterone a hormone required for implantation of zygote and its nourishment is being assessed for it adequate.

Endometrial biopsy: A tissue from uterus is taken and investigated in lab for its structure and normality.

Hysteroscopy: This is the visualization inside the uterus via camera.

Clinical evidence of normality of the internal genital tract

Hysterosalpingography: Via camera visualization of uterus and tubes called fallopian tubes

Transvaginal sonography. As discussed above

Hysteroscopy. As discussed above

Laparoscopy. Via camera visualization is called laparoscopy
