Question : sir i have very serious problem "in starting of menses i have very much pain very much.which i can handle. please give me solution.i am waitng for ur answer.
Answer : There are two types of dysmenorrhea (menses pain) primary and secondary. You need treatment. You are advise to consult your lady doctor for treatment.

Primary Dysmenorrhea

Pathophysiology
Primary dysmenorrhea occurs during regular ovulatory cycles. Women with primary dysmenorrhea have increased activity of the uterine muscle with increased contractility and increased frequency of contractions. Prostaglandins are released during menstruation due to destruction of the endometrial cells and the resultant release of their contents.

Release of prostaglandins and other inflammatory mediators in the uterus (womb) is thought to be a major factor in primary dysmenorrhea (Wright et al. 2003). Prostaglandin levels have been found to be much higher in women with severe menstrual pain than in women who experience mild or no menstrual pain. Drugs which inhibit the production of prostaglandins, such as the non-steroidal anti-inflammatories (NSAIDs) Naproxen, Ibuprofen and Mefenamic Acid, can provide relief for the discomfort and other associated symptoms of excessive prostaglandin release, such as nausea, vomiting, and headache.

Clinical Features
The cramping associated with dysmenorrhea usually begins a few hours before the start of bleeding and may continue for a few days. The pain is usually described as being in the lower abdomen, possibly radiating to the thighs and lower back. Other symptoms associated with primary dysmenorrhea are nausea and vomiting, fatigue, diarrhea, lower backache, and headache.

Treatment
Nonsteroidal anti-inflammatory drugs (NSAIDs), such as ibuprofen and naproxen, are very effective in the treatment of primary dysmenorrhea (Andreoli et al. 2004). As earlier stated, their effectiveness comes from their ability to inhibit prostaglandin synthesis. However, many NSAIDs can cause gastrointestinal upset as a side effect. Patients who cannot take NSAIDs may be prescribed a cyclo-oxygenase (COX) inhibitor. Oral contraceptives are also sometimes used since they reduce menstrual flow and inhibit ovulation.

Oral contraceptives are second-line therapy unless a woman is also seeking contraception, then they would become first-line therapy. Oral contraceptives are 90% effective in improving primary dysmenorrhea and work by reducing menstrual blood volume and suppressing ovulation. It may take up to 3 months for the oral contraceptives to become effective. Norplant and Depo-provera are also effective since these methods often induce amenorrhea. (wikipedia)

WHAT DO I NEED TO KNOW ABOUT NORMAL MENSTRUAL CYCLES?
The menstrual cycle is the process that prepares the uterus for pregnancy. If fertilization of an egg does not occur, menses (the period) will begin. Females usually begin to have periods around age 12 (menarche) and stop having periods around age 50 (menopause). The average length of the menstrual cycle is 28 days, however in reality the normal menstrual cycle may vary in length from 26-35 days. Menstruation (bleeding) usually lasts 3-5 days. Menstrual cycles have two phases. The first, or proliferative phase, may vary from 13-20 days and ends when ovulation occurs. This phase varies from one woman to another and may also differ from month to month. The second, or secretory phase, begins after ovulation and ends when the period starts. This phase generally lasts 14 days for all women.

Question :
Hi, my problem is that :during menses in the start means first 3 or 4 days i dont feel any pain at all very normal bleeding without any pain everything ok in first four days but in the end of menses when they r going to be in the end i feel much pain i want to ask why because mostly girls told me they feel pain in the beginning but in my case i feel pain in the end like 5th or 6 th day .kindly reply to my question. thx a lot. bye
Answer : Pain during menses is called dysmenorrhea. For treatment you are advise to consult a lady doctor.

What causes dysmenorrhea?

The cause of dysmenorrhea depends on whether the condition is primary or secondary. In general, females with primary dysmenorrhea experience abnormal uterine contractions as a result of a chemical imbalance in the body (particularly prostaglandin and arachidonic acid - both chemicals which control the contractions of the uterus). Secondary dysmenorrhea is caused by other medical conditions, most often endometriosis (a condition in which tissue that looks and acts like endometrial tissue becomes implanted outside the uterus, usually on other reproductive organs inside the pelvis or in the abdominal cavity - often resulting in internal bleeding, infection, and pelvic pain). Other possible causes of secondary dysmenorrhea include the following:

  • pelvic inflammatory disease (PID)
  • uterine fibroids
  • abnormal pregnancy (i.e., miscarriage, ectopic)
  • infection, tumors, or polyps in the pelvic cavity
