Male Infertility And Semen Analysis: A Simple And Basic Understanding

                                                                                                                                                
Iss forum par bohot say readers semen analysis say related questions kartay hain. Iss article ka basic purpose yeh hai keh readers ko semen analysis kay baaray mein aik simple aur basic understanding dee jae takeh woh apna semen analysis ka baaray mein atleast basic understanding haasil kar sakain. Meri koshish ho gee keh mein complex medical terminology aur detailed analysis  ko avoid kartay hoa  sirf itni information doon jo aik aam reader aasani say samajh sake. 

Semen analysis kiyoon karwaya jaata hai: 

Most cases mein semen analysis infertility ke evaluation ka liya kia jaata hai. Takreeban 30% infertile couples mein problem abnormal semen/sperms ke hoti hai.  Iska illawah woh males jin ki vasectomy ( male birth control surgery) hoi hai unka bhee semen analysis karwaya jaatay hai takeh yeh patah chal sake keh vasectomy succesful hoa hai ya nahin.

How to collect semen for semen analysis: 

Proper results ka liya zaroori hai keh semen collection say pehla approximately 3-5 days tak sex ya masturbation nah ke jae, otherwise results accurate nahin hon gay. Zyadah tar cases mein semen lab mein aik clean container ( jo lab provide karti hai) mein masturbation kay zarye collect kiya jaata hai. Kuch log yeh prefer kartay hain keh lab mein sample collect karnay ka baje ghar mein sample collect karain aur phir lab tak pohchaian. Iss sorat mein kuch basic baaton ka khayal rakhna ke zaroorat hai. First yeh ke agar aap kisi container mein sample collect kar rahe hain to woh container clean ho aur uska lid tightly band hota ho. Agar aap container ke bajae condom mein semen collect karna chah rahe hain to yeh bohot zaroori hai keh pehla condom ko wash kar lain. Yeh iss liya zaroori hai keh condoms kay ander spermicidal jelly hoti hai jo sperms ko kill kar daiti hai, iss liya aap ka results sahih nahin aae gaa kiyonkeh sperms dead ho gae hon gay. Jab sample collect kar lain to usko jald say jald lab tak     pohchain ( within 30 minutes or so), aur sample ko refrigerate nah karain, aur koshish karain keh sample body temperature par rakhain. Issi tarah direct sunlight say bhee sample ko bachaian. 

The results of semen analysis: 

Semen analysis mein generally kuch basic values check ke jaati hain jo mein describe kar raha hoon. Kuch labs isska illawa bhee kuch doosri cheezain mention kar sakti hain, magar aam tor par jo cheezain report mein mention ke jaati hain woh yeh hain: 

1). Semen volume: Generally normal semen volume approximately  1.5-6.5 ml hota hai.

2). Liquefaction time: Yeh woh time hota hai jis mein semen aik thick liquid say aik watery liuid mein change hota hai. Normal liquefaction time approximately 20-30 minutes hota hai. Agar koi infection ho to liquefaction time increase ho jaatay hai.

3). Sperm count: Generally, yeh million/ml mein show kiya jaata hai. Normal sperm count approximately 15-20 million sperms/ml hota hai.Isko sperm concentration bhee kehta hain. On the other hand, total sperm count pooray sample mein sperms ke quantity ko show karta hai. Generally 40 million sperms  per ejaculate normal consider kiya jaata hai. Bohot say logon ka sperm count normal say low hota hai, magar agar unki sperm motility bohot acchee hai to woh low sperm count ko compensate kar sakti hai. 

4). Sperm motility: Motility means the ability to move. Intercourse ka baad sperms ko move kar keh egg tak pohonchna hota hai, aur jaldi phonchna hota hai. Iss liya sperms ke movement bohot important hai. Magar unfortunately, move karna ke ability har sperm mein nahin hoti, aur movement hoti bhee hai to kuch mein fast hoti hai (good) aur kuch mein bohot slow( bad). Generally, sperm motility ko do mukhtalif tarah say describe kiya jaata hai: 

 Grade A, B, C or D system:  Grade A best quality ke movement hoti hai, aur grade D woh sperms hota hain jo move nahin kar paatay. Generally 50-60 % sperms grade A ya B kay hona chahyain. 

 0,1,1+,2,2+,-3,3,3+,4 system: 0 means sperms that can't move, aur 4 means fastest moving sperms. Normally, 50-60 % sperms atleast  2+ ya 3 hona chahyain.
 Agar kisi ki sperm motility low hai magar sperm count bohot achaa hai to yeh bhee low sperm motility ko compensate kar sakta hai. 

5). Morphology ( shape): Sperms kay kae different shapes hota hain, aur har sample mein aik barri taadad abnormally shaped sperms ke hoti hai, for example pin head, double tails  etc. Abnormally shaped sperms ka liya egg ke fertilize karna mushkil hota hai. Morpholgy bhee 2 different systems ka zarya describe kee jaati hai: 

  WHO criteria: Iska hisaab say agar approximately 30 % sperms normal shape ka hain to sample normal consider kiya jae gaa.

  Strict criteria: Yeh aik naya system hai, aur isska hisaab say even agar atleast 5 % sperms bhee normal shape ka hain to sample normal consider kiya jae gaa.
    Generally WHO criteria is more commonly used. 

6). Fructose: Fructose aik khas tarah ke sugar hoti hai jo sperms ko energy provide karti hai. Semen mein fructose ke mojoodgi zaroori hoti hai( normal valua approximately 3mg/ml). Agar sample mein fructose nahin hai to yeh abnormality consider kee jae gee. 

7) PH: PH kisi cheez ke acidity ya basicity ka meausre hota hai. Semen ke normal PH value approximately 7.2 -8 hoti hai. Agar value iss say zyada ho to infection ke possibility consider ke jae gee. Agar value iss say kam hai to internal blockage ( seminal vesical blockage ) ke possibility consider karni chahye. 

8)WBC/Bacteria: Agar semen mein WBC ya bacteria show ho rahe hain to yeh infection ke posssibility show karta hai. Bohot small quantity mein WBC present ho saktay hain normally. 

9) RBC: Generally semen mein RBC nahin hotay, agar RBC show ho rahay hain to injury/bleeding ke possibility hoti hai. 

What to do if semen analysis comes back abnormal: 

Agar aik semen analysis abnormal aa jae to koi bohot zyada pareshani ke baat nahin.Agar aap ka semen analysis abnormal aaya hai to sab say pehli cheez yeh karni hoti hai keh repeat semen analysis kiya jae. Iska liya best yeh hai keh 2-4 weeks ka waqfa say 2 martaba aur semen analysis repeat kiya jae kiya jae. Yeh baat zehen mein rakhain keh tobacco use aur caffeinated drinks sperms ka liya harmful hoti hain. Issi tarah bohot zyada heat bhee testis aur sperms ka liya harmful hoti hai, iss liya bohot tight clothing avoid karain. Agar aap ka repeat semen analysis bhee abnormal aaya hai to aap kay doctor ho sakta hai keh kuch aur tests bhee order karain, for example LH levels, testosterone levels, anti sperm antibodies,  testicular ultrasound, ya phir chromosomal analysis etc etc.

Question 1) Sperm Morphology.. Can you please help me learn this...
For example :
sperm morphology

Sperms with round head 60%
Sperms with double head 15%
Sperm with pin head 10%
Sperm with tails 80%

how can we tell, that the oval head sperm here is the double head?
and that it is considered Normal?
Also psychdoc bhai... in these number and descriptions how can we
tell what sperm is in concentration or progression?

Sperm morphology and motility:

Thank you very much.

Regarding sperm morphology, there is a great variation in sperm shapes, but generally oval headed ones with a single, non coiled long tail are considered normal. Sperm morphology is assessed through microscopic examination of the semen sample. When evaluating for infertility, sperm morphology is of less importance as compared to sperm count or motility.


For progression ( motility) assessment, there are a few different ways of doing it. The manual motility estimate is done by microscopic exam of the wet mount of the sample.The examination is done by bright field microscopy, or for even better results, phase contrast microscopy or differential interference contrast can be used.
Another way of assessing motility is through a computer, and the technique is called Computer Aided Motility Analysis .

I hope I haven't confused you with too much detail.

Regardless of terminology.. I am extremely excited and happy to find you helping me in this subject..

So if the Morphology is less than 30%, of normal form, does that mean that the sperms have abnormalities?
If sperms would have abnormalities would that mean that.. ofcourse through microscopic way only but you are able to tell which form these sperms are in beside oval, like double headed, round headed or long coil tail pins?

I am glad to be of help to you. Anytime!

As I had mentioned in the article, morphology is assessed using two different systems. If the WHO criteria are used to assess the sperm shape then yes, if less than 30% sperms are of normal shape, than it would be considered abnormal.
And yes, the different abnormal shaped sperms like the double headed, round headed, broken tails etc can all be detected through microscopy.

quick question to you...

It is my understanding that high PUS CELLS are considered possible infection if found in one's semen, but.. one of the posts of a doctor.. id, i wont name, said that if PUS Cells are high, can just mean more solidarity in blood, rather infection..

whats the truth in this?

You are very welcome.
Please note that every semen sample will contain some pus cells or WBC. Generally, anything less than 1 million/ml is considered OK. It is also to be noted that immature sperms look very similar to WBCs, and most labs on normal testing, can not differentiate between immature sperms and WBCs, therefore a sample can erroneously be interpreted as high in WBCs. Other factors to look at would be how high the PH is. Clinical correlation is very important when considering the possibility of infection/inflammation.
Please clarify what you mean by "solidarity in blood".

WBC in semen:


Ali bhai,

Thanks, but all greatness is for Allah. I am only a very humble human.

As I described above, a small amount of WBC are normal in semen. Yes, the WBCs are the body's defensive force that help fight germs/infections.

A normal amount of WBCs in semen would be anything less than 1 million WBC/ml. In terms of how it translates to WBC/hpf, different labs have slightly different interpretations. Some consider a normal value to be less than 7/hpf, some use a slightly higher number. Almost every lab uses it's own parameters and it's best to check what is the normal range that is used by that particular lab.

Is PUS Cells, WBC?

Do forgive me for this question, its just that... im still learning this, dear friend..
WBC is indeed PUS Cells or is PUS Cells different?

normal value is less than 7/hpf which means any number of PUS Cells 1 through 7 is normal and does not attract attention? Anything more, can be a sign of infection? So we should recommend a repeat test, if the repeat test show same numbers, higher than 7, than a physician, lab doc can request :

"LH levels, testosterone levels, anti sperm antibodies, testicular ultrasound, ya phir chromosomal analysis etc etc."

Pus cells:


Ali bhai,

Yes, pus cells are a subtype of WBCs.
If infection is suspected than the doctor will either start antibiotics, or order semen culture first before starting the antibiotics, depending upon the clinical situation.

Testosterone levels, FSH,LH etc would be ordered generally when low sperm count/motility is present as part of infertlity evaluation, not when infection is suspected.

Dear Psychdoc bhai:

In the thread in which I had you intervene, i wanted to confirm, that if the Volume was more than 2ml, like 4ml... and the sperm count is 20million.. than it means 2 multiply by 4 or what? meaning the person is being told he has less sperm count by it being 20 million, but in actual its 80million?

Aap ka doctor aap ke condition kay baaray mein sab say behtar mashwara de saktay hain, kiyonkeh unhon nay aap ke detailed history lee hai aur physical exam kiya hai. Agar unka mashwara hai keh aap ko kuch test karwanay ke zarrorat nahin to chances yehi hain keh aap ko aisi koi problem nahin. Kabhe kabhaar low testosterone levels ke wajah say bhee erection mein problem ho sakti hai aur iska illaj testosterone injections hota hain ( jaisa aap kay doctor na bataya), baharhaal yeh bohot common reason nahin hai erectile dysfunction kee.Aur agar testosterone level low hai to uska aur bhee symptoms/signs hota hain, for example lack of facial hair, lack of pubic hair etc. Behtar yehi hai keh aap apna doctor ka mashwara par amal karain.

Sperm concentration ( number of sperms /ml) is considered normal if it is 15-20 million sperms/ml. Total sperms in the ejaculate would be calculated by number of sperms/ml multiplied by the volume of semen in ml.Normal value would be approximately 40 million sperms in the ejaculate.

Heat and testicular function:


Testis are effected by heat. Heat generated by a laptop can effect testicular function, particularly with prolonged exposure.

 Sorry for the late reply.

 Spermatogenic cells are immature sperms, or in other words they eventually form mature sperms. Some immature sperms can be present in the semen normally.

Ali bhai,

I admire your hunger for learning, and I am glad that I am atleast of some help to you in the process.

PH is a measure of how acidic or basic a substance is. The lower the PH the more acidic the substance will be, and the higher the PH, the more basic it will be. The normal PH of semen is 7.2-8. Instead of just mentioning the word alkaline it is more important to mention the exact PH in numbers. And as I had mentioned in the article, higher PH ( more alkaline) would point towards the possibility of infection.

Consistency is described in several different ways by different labs, for example thick, thin, semi liquid etc. However, consistency is not of much importance anyway.
