mostly this condition in males is of unknown cause(idiopathic) to which it is said that an underlying genetic (chromosomal/gene material/hereditary) may or may not be present..
other second most common cause is varicocele(dilated tortorous / more diameter veins) of testes which may or may not be noticed by the person himself and require examination by doctor himself and if required a few investigations..
it may be due to hormonal disturbance in body
presence of antibodies against sperms(body cells defence against own sperms)

treatment:
it depends on underlying cause...
like varicocele demands surgery , opening of blocked tubes , control of infection if present or inflammation control , hormonal correction etc..
and ifno cause is found
mostly these days three types of treatment are done if fruitful
hormonal
anti-oxidant therapy .... giving vitE , selenium and vit C
also certain anti-inflammatory agents are combined...
plus general guidelines for healthy sperms:
avoid smoking , excercise daily , donot be overweight , fresh fruits and vegie , avoid warm water over testes, loose undergarments etc....
